Book Your Appointment
Pay Visit Fee Online
Select the "Consent" Tab at the top of the page.
Read, fill out and Sign the Consent form. Only Required to sign once yearly.
Click the Send Button. 
After you have chosen your desired date and time for your appointment, you will prompted to pay. Click the 'pay now' box that appears to confirm your appointment time.  Click the big green button at the bottom of the home page at your appointment time to log in for your appointment. 
Select the "Book Online" Tab at the top of the page. Click the "Book It" button and select the appointment time that works best for your schedule. You will receive an email confirmation and check-in instructions.

Note: All Three Steps Above  Must Be Completed Before an Online Visit Can Be Offered.  

No Computer? No Problem!  Virtual Medical Visits Can Also be Conducted over the telephone.
